Adapalene (Differin Gel 0.1%) is a third-generation synthetic retinoid, insurance-covered for acne in Japan. Selectively binds RARβ/γ subtypes to normalize follicular keratinization and prevent comedone formation. Significantly less irritating than tretinoin with superior photostability. Rated highest recommendation (A) in Japanese Dermatological Association acne guidelines.
작용 기전
Adapalene selectively binds RARβ/γ: ①regulates follicular keratinization to prevent abnormal cornification (comedolytic), ②inhibits AP-1 to suppress IL-1β/IL-6/TNF-α (anti-inflammatory), ③dissolves existing microcomedones preventing progression, ④modulates TLR2 expression. Low RARα binding explains reduced irritation vs tretinoin.
적응증
기대 효과
Comedone reduction starts at 2-4 weeks. At 12 weeks: 50-60% reduction in inflammatory lesions, 60-70% in non-inflammatory. 65% of patients prefer adapalene over tretinoin (less irritation). Synergistic with benzoyl peroxide (guideline-recommended combination).

위험 및 부작용
Initial retinoid reaction (dryness, peeling, erythema, burning) common in first 2-4 weeks, usually subsides by 4-6 weeks. Teratogenic - contraindicated in pregnancy. Photosensitivity requires sunscreen. Avoid eyes/mouth/mucosa. Moisturizer improves tolerability.
