About This Treatment
Motiva is an Establishment Labs silicone gel-filled breast implant family featuring a SmoothSilk shell and Ergonomix-style dynamic-shape behavior. It gained traction earlier in Europe, Korea, and other international markets, and the U.S. FDA approved SmoothSilk Round and Round Ergonomix for breast augmentation on September 26, 2024. In its January 10, 2025 trends coverage, ASPS also highlighted Motiva as one of the breast-augmentation technologies generating current buzz around softer, more natural-looking results.
Japan publicly announced PMDA approval on November 10, 2022, making it a high-signal cross-regional augmentation option rather than a U.S.-only novelty. Even so, this remains implant surgery, not a casual in-office procedure. Size planning, implant plane, soft-tissue coverage, prior capsular issues, future breastfeeding or imaging needs, and the possibility of later revision all need to be part of decision-making.
Mechanism of Action
A silicone gel implant is inserted through an inframammary, transaxillary, or other surgical approach and placed in a subglandular, subfascial, dual-plane, or submuscular pocket to increase breast volume and shape. Ergonomix-style implants are designed so the gel redistributes more dynamically with position change, aiming for a more bottom-weighted contour upright and a rounder distribution supine. The final look depends not only on the implant but also on chest-wall anatomy, tissue stretch, gland and soft-tissue thickness, and pocket execution.

Expected Results
Volume change is immediate, but the practical result is usually judged after 3-6 months once swelling settles and the implant pocket relaxes. Published Motiva cohorts report high satisfaction and generally favorable short- to mid-term safety, but breast implants are not lifetime devices. Future revision may still be needed for capsular change, malposition, rupture, aesthetic preference shifts, aging, or weight change.

Risks & Side Effects
Hematoma, infection, wound problems, seroma, pain, capsular contracture, asymmetry, malposition, bottoming out, rippling, sensory change, imaging follow-up needs, rupture, and reoperation are all possible. Breast implants are not lifetime devices, so later exchange or removal may be necessary. Even with smooth-surface devices, standard implant warnings and long-term surveillance still need to be discussed.
